Rotary axis identification needed

I bought this 4th axis for a job but cannot find any info on it. There is no nameplate except it has "615-36-3-5" stamped on top. It has an AC motor drive, black wrinkle finish, 15" diameter faceplate, and air brake. I've posted some pictures so if someone recognizes it and can give me any info, I'd really appreciate it.

I bought this on eBay at least 20 years ago, so it has to be pre-1998.
