OEM750X Help needed

I am installing 4 oem750X drivers along with the power supplies on my CNC and need some advice,

1 - Do I need to remove the indexer? and jumper out pins 1-5?
2 - Will the 750 work with a 100Khz Mach3 USB BOB or should I order the C10 BOB?
