Do you have two home switches?
Parallel port or motion controller?
On the initial homing of my machine I have no problems and I'm able to run a job successfully. However lately whenever I try to re-home the machine I'm having the following problem, does anyone know what causes this?

Yes, the slave "A" is on a separate home switch.
Parallel port.

DO you have "Home Slave with master" checked in General Config? Uncheck it if you do.

It is unchecked, it wouldn't home originally if it were, or at least I don't think it would.

Uncheck "A axis is angular" on the left side. Not sure if that will help or not?

I'm using your 2010 Screenset so I'm guessing you have the code for that.. although it happens with 1024 also.
I'm just not certain if I'm looking for an electronic problem or a software problem.

It works every time the first time you start Mach3, but after that it no longer works?
Are you just clicking the home button once, and it homes one side multiple times?
Very strange if that's the case. My first guess wouold be a corrupt .xml file, but I really have no idea.

You might want to change the homing macro to use "RefCombination" There's info in the 2010 Screenset manual on how to do it.

I'll try to do some testing tomorrow and be more specific as to when and what is happening, thanks for the suggestions.
I may also post on the Mach forum to see it they think it might be software related..

Since I'm using your screenset I assume I should be using using the 2010.set and not the 2010-4.set since my 4th axis is slaved, correct?

I was just about to post the macro for you.
What you have is correct.
But it really doesn't make any sense that it works correctly the first time, but not after that. But I've always used RefCombination, so hopefully that's it.
What version of Mach3 are you using?
You can use any of the .set files, but since you don't have 4 axis, if you use the 2010-4, the A DRO's axis will not display anything. So yes, you really want to use the 2010.setSince I'm using your screenset I assume I should be using using the 2010.set and not the 2010-4.set since my 4th axis is slaved, correct?

I think it means that the switch on the side that's not moving is already triggered before you click the ref all button. Open the diagnostic screen and you should see the LED before you home.

So, I'm trying to figure out what's up and I may be stumbling upon something just not sure what....
When I powered up this morning I noticed that the Machine co-ordinates for X and Y were both 0,0 and the gantry was no where near X and Y 0,0. So, I started looking at the home and limits settings and didn't notice anything strange. I tested all the Home switches and they all work accordingly and I also pulled the control panel thinking maybe a lose wire but nothing.
I then checked the soft limits button since it didn't make sense why the machine thought it was at 0,0 and I'm not sure what happened but things are working again, I must have missed a button click or something..ugh!!???
So here's a video using the default "Ref Al Home" after the miracle fix and it works repeatedly without issue..
Any ideas, somehow I think its tied to the Soft Limits, just not 100%.

Based on the error message, it seems like one of the switches was somehow activated. Not sure what caused that, but if it happens again, check the diagnostics page.
Is that a SuperPID on the side of the gantry? If so, the Heat Sink should be enclosed so that you can't touch it, as it's possible to get a 110V shock from it if something goes wrong with the board.

Just ran into this issue again, strangely enough my Z axis had tripped the limit switch and I kept over looking it..
