Check the run-out on your spindle. If the bit is not running concentric this will happen. Is it possible you can do a finish pass?

What exactly are you trying to make? You will almost always want a "continuous surface" tool if you want smooth surfaces with glass. That looks like something you might use for granite. Glass really needs to be babied. Those flutes are creating micro-impacts on the glass and chipping it. Glass can almost always be cast easier than machined. I guess that leads me back to the first question. Are you trying to make glass shapes or precision glass shapes?
