Biggest Challenges with Workholding / Fixtures?

What are the biggest workholding challenges everyone is dealing with. If you had anything you wanted what would be looking for with regards to hydraulic or manual CNC fixtures?

A full size auto-segmented (it only sucks where I place a part) vacuum table that makes no noise and doesn't let go of parts when you accidentally break the seal :-)
