Post Yama Seiki AV-610 SV5136 FSSB Problem with 4th axis!

We recently purchased a nikken CNC 180 rotary table for our 2012 Yama Seiki with Fanuc Oi-MD control. We installed the motor and amp suggested by nikken to install in rotary. We purchased the cables from Yama needed from the table to the amp and etc. We enabled all the setting nikken told us to and Yama as well. We are know getting a SV5136 FANUC error. It says number of amps insufficient. We have reached out to Yama but not very helpful. Any suggestions? Here is my layout below and I attached photos.

    Fanuc amp for 4th A06B-6117-H103
    Thanks in advance
