We had a board go out on our Miyano BND 34s with a Fanuc 21-T control. With the Help of Fanuc and Local Miyano service, We have the machine up and running, but it doesn't generate a Radius. We bought the machine used and found out that we didn't have the parameters for the machine. fanuc entered parameters they thought would work. Miyano hasn't been able to help.
We have a program that uses a G03 X.75 Z-.05 R.05. And the machine does a big radius away from the part on the Z axis and then way above on the X axis and stops at the end point but doesn't come close to the part.

first of all i should try to run the cycle with SINGLE BLOCK activated and check the values onISTANCE TO GO page and when start the radius check where is going and make an analogy
second ,try to see if you do not have some scalling factor activated or some mirror in operator page
make a small video showing us the screen with the all coordinates:absolute,machine,distance to go and also show us what it has to do and what it really does.
maybe with this info we can have some ideas of what is going on.
also paste here the 9000 parameters to see hwta options you have activated.
