Alarm when calling same tool. Fanuc 18i-m Mitsubishi M-V5Cn-L

hi drdos,
could you please help me on cincinnati retofited machine with 18i-MB control, we can select MDI,AUTO,AUTO mode on MOP. i tried to replace MOP & I/O board but same proble i guess problem in ladder. i attached ladder below please check and help me.

Looks like Keep Relay K4.5 is off.
Please see picture attached, to make sure I highlighted correct keep relay.
