Puma 4100 gear boxes

Was looking to see if anyone else has had issues with spindle gear boxes failing on a Doosan 4100? We have had two machines fail within the last year. One of them being last week. Both boxes were full of oil so that eliminates that question. Both machines are older too.

Just trying to get some ideas on what would be causing this issue.

I used to work for them, and never heard of a spindle gearbox failing.
Were they crashed hard in "Z"?
What exactly happened?
