Default Record 120 PLC Error 125 : Tooling Error Suggestions?

I am getting PLC Error 125 whenever I use attempt to call a 1/8inch end mill or 1/4 inch end mill in a program, I was able to use the V carve bits, but that is now causing an issue. Anyone have any suggestions for fixes or even a method of narrowing down the reason for the error?

Thanks Much, everyone here has helped me so much already.

To anyone coming across this issue :
It can be caused if there is a tool in the spindle when you call for a tool change. Or rather, the cnc sees the electrical signal for a tool being present, even though it believes the spindle is empty

In my case, this was caused by a tool draw bar sensor misalignment in the spindle.
