Please help! Broke my Y axis..

My machine (X3) lost its Y origin, and when I went to check the machine it was all messed up. The part at the end of the Y-axis ball screw was broken.. Ball bearing everywhere.. not good.. I have it appart, and this is the piece I need:

Can someone please tell me what this is called and a part #. I am in real trouble, I need to get this thing going asap.

i would add limit switches to prevent over travel crashes in the future,saved me on my
x2 couple of times

And yes it''s totally my fault, my limit switches are in a box, I never put them on. What a big mistake.
