PCB-GCODE Setup General Options

Hello, I am hoping some folks here use a work flow with Eagle - Pcbgcode and could shine some light on my problem.
I'm new. I have studied pcbgcode and still can't get it right. Less than 10% of traces are output. Also it shows tool path for milling the board outline, I have the top etch as a ground plane, maybe it is cutting my ground plane, lol.
I am in no way blaming softwares, I am sure i didn't check a box somewhere.
eagle pic is what it should look like.
cambam pic is what I get.
Anyways, if someone has a minute please download the board file and see what pcbgcode outputs for you, or take a look at the pictures to see if maybe you recognize the problem.
NOTE: Rename brd file.dxf to: brd file.brd This is an upload work-around, since the Eagle .brd file extension is an invalid type.

pcbgcode 3.5.2.11
cambam 0.9 for viewing paths and editing
Mach3
all traces on board are 10mil, clearance 8mil
my setup for pcbgcode is:
default .001
max .008
step size .005
tool size .007

I have seen your name all over the internet and I hope you continue for the sake of others like me who have wondered just how far they can hurl their laptop.
The link you gave is one of the most practical I have seen. I had success in less than 3 minutes after clicking it. Let it be a guide for others...

You are welcome. Phil's site is what helped me get up to speed with PCB-Gcode and double-sided milling.
