The brake there should be a cable on the housing of the old one.
The bearing are Star/Rexroth bearing cars....you should be able to find a number etched on the old car. You will have to find the Rexroth distributor for your area, don't buy from ebay a lot of the pictures are generic. They will also have the stripping.
Check with Motion Industries, Bearing and Drives for the pulleys.
