Hitachi Seiki HG630 II Alarm 2237

My machine is repeatedly getting this alarm during machining. 2237 spindle automatic mode rotation check error . The speed / orientation sensor is not damaged, but i can't determine why it doesn't always give the rotation detect signal to satisfy the ladder logic.
