Mazak HTC 400 w/ 1 degree indexer

We have an older HTC 400 that we are having problems with the indexer. The indexer is stopping when given any command greater that 30 degrees (1-30 degrees works fine). Indexer stops and control needs to be reset - watching indexer position while returning to home, I do not see a value greater that 30 degrees.

We do get 213 indexer driver malfunction when trying to MDI to 31 degrees

What does the manual say a 213 alarm is? Maybe the axis has lost its range of travel and needs to be rehomed or setup.
