Unhappy Mazak v-15n tool index issue

We have this mazak with a mazatrol m-2 control. My operator found out the hard way last week that all tool heights in the tool index had somehow been altered -.080". So the machine thought all the tools were shorter than they actually are. This somehow happened on the last setup of the parts he was making. Our question is has anyone heard of this happening, or even have an idea of what caused this? We're really worried of having this happen again now. I could understand operator error if it was just one or two tools, but every single tool height in the machine was altered. Any ideas are appreciated.

Altered how. Offset actually changed value or did you have to comp all the tools to make a good part?

All of the physical tool height numbers were altered. Operator had to go back and touch off tools and reset the tool height in the tool file.
