When has this ever happened to you?

The other night I was taking a finish pass, side milling through a 5" steel tube. While i was deburring some parts i heard it crash and thought the endmill had broke. The endmill and tool holder had fallen out of the machine spindle, it didn't trip any alarms and the machine was moving right along. What i'm most curious about is when i looked at the tool holder the pullstud was badly bent, and i was wondering what other instances you've seen a pullstud bend?

The pullstud was probably bent because the tool holder hit something on the table.

You might want to check out the jaws that actully hold the tool in the spindle, had this happen with a comet vmc,it would break the pull studs becouse it wasn't pulling even,(one was broke).
