spindle rebuild failure

my intermac 1500 ot has had its spindle rebuilt the collet replaced 3 times and the tools are still not locking in and causing ripples in the stone shaking and vibrating and the router or finger bit will not even run unless its brand new and even then it will only run for 1 or 2 sinks before dulling out
