Source for pre-assembled 4-axis controller

A while back I found a source for a pre-assembled and tested controller with power supply based on the Xylotex kit. Of course, now I can't find it again. What I particularly liked about this pre-assembled kit was that it came with inputs for limit switches (which the Xylotex does not) and a jog wheel or pendant controller for jogging the spindle.

Anyone know where I can find that kit, or another source for pre assembled controllers?

I don't know if this is the one you found or not, but CandCNC sells such a package. I use one of their Mini-IO breakout boards and I really like it.
