Post W axe run away on Lucas Milling machine with Dynapath 2000M

We have a Lucas milling machine 30DC-LM4 50 M retrofit in 2005 with a Dynapath Delta 2000M control and bulleting 1388 AB servo controllers.
The problem is that the “w” axe run away as soon as the E-stop is cleared, causing the machine to stop.

Before start the machine, the servo controller was tested separately and works fine; even we put zero reference and adjusted the offset so the “w” shaft does not move at all.

With the fail still present, we put the dynapath in TM5 mode, enabled the servo controller, use an external reference and moved the w shaft both directions. We check the encoder reading any time.

We adjust the encoder so the reading was indicating the real position in “w”.

After the failure, the EST led of the dynapath controller lit so the upper red led of the board besides the port board. I do not have a manual to see what those led mean when a failure happens.

Could you tell me what might be the problem cause?

    A guidance could be great.

    Reinaldo.

Usually, when a servo axis "runs away" it means that the encoder has failed. Try repairing or replacing it.
