Tool path of 1 square mm slant surfaces require optical quality

I am working on a job require to have thousands of 1 mm square slant surfaces in an array on a plane board. The square surfaces require to machine to optical quality. A sample square ABCD might have following coordinate:
A (top left, 0, 1, 0)
B (top right, 1, 1, 0.5)
C (bottom right, 1, 0, 0)
D (bottom left, 0, 0, -0.5)

The adjacent sides between the squares require to be sharp.
I need the information about how to machine this square array. The tool path to work on each square, the tools required, machine speed.
I need to machine on acrylic board for prototype and then negative on metal board for hot press production.
