Problem- 403 SERVO ALARM !!!

Hello.
Any one help me with 403 SERVO ALARM on fanuc OMC system.
I checked SERVO drive is ok, cable from servo card to servo drive is ok.
I thing servo card was broken but i not sure because i do not have experiment with fanuc OMC system.

Thank you.
I check voltage to driver is ok, 210V 3 pharse and 100V for power control.
I swapped drive Z and Y but machine still display this alarm 403 SERVO ALARM.
Do you thing servo card was broken ?

This is alarm for 4Th axis. You have to turn off option for 4Th axis

i hav sam alarm 403 im change the tensistor ,bat sam alarm .
