Fanuc O-M Alarms #1006 and #409 on crt

Need Help!!!

When giving machine control any spindle command,
We are getting alarms #1006 and #409 on crt and alarm 12 on Spindle Amplifier module # A06B-6088-H215#H500

This is a common alarm for spindle amps. If your motor and motor power cable meg fine the issue is with the spindle amp. Check out our website, www.tristarcnc.com for pricing.

I do not have a meg meter to check with, so I disconnected the U , V , W , wires from the spindle amp and gave it only an S1500 command and still got the same alarms.
Would this indicate issues are with the A06B-6088-H215#H500 Spindle Amplifier Module and not the cable or spindle motor?

The problem is within the A06B-6088-H215#H500 spindle amp.
