Turret Index Only Once In Auto

DEAR ALL,

I have a yam make turning m/c with 12 turret post,

Control Fanuc 3T
M/c MAKE : CK-1-3T
M/C MODEL: A02B-00580B501
SR. NO. 71-0784
YAM NO. M-7022
12 TOOL TURRET

In this m/c all function are running except turret indexing in auto cycle.
As turret command is executed, turret index only once , no any alam displayed , dignosis bit 7 of 712 get high & after that no auto operation possible. All the Prox. & micro sw signal are ok.
After control off/on auto operation possible but except turret indexing.
