Fadal VH-65 rotary, face plate layout.

I'm trying to build some tooling that I can use on my VH-65 head, and would really love it if somebody had the layout of the face plate. If I remember correctly, it has a 1.625" center locating bore and it also has a 135mm 1/4-20 bolt circle with 6 bolts. It has 4 t-slots that can use a 3/8 bolt.
Then it ALSO has a couple other bolt hole patterns. I don't want to tear down my current job to measure this up for a future job that probably won't run for 2 months, but I sure would love to get that face plate all laid out so as I design various things, I can make sure I get the proper patterns laid out.

So, does anybody have it, and if not could you measure it out for me and let me know? If this attachment comes thru, you can the the #3 item has what looks like 3 different bolt hole patterns on it.
That is what I'm hoping to find.

The center bore for locating, is it 1-5/8" How deep can a plug fit into that ?

1-5/8 yes counterbore is .25 deep
