Need A Quote- Need a simple part made

I need a simple part that consists of a 1/2 thk aluminum plate (6061-t6, cast aluminum jig plate, or eqiuv) that is roughly 18"x15". There are 16 counterbored holes for cap screws. I was going to do this myself but did not want to take a chance with the hole locations.

I've attached a drawing for the part, let me know if further information is needed. Please quote one piece and shipping to tonawanda, NY 14150. Thanks.
