RFQ: ABS Sheet CNC or Waterjet?

Need a quote on the attached housing cover.

This is a very simple part, but will need approximately 5,000 of them over 3 years in 1,000 pc lots once prototyping is completed. CNC or waterjet seems like it may be the best way, but I am open to alternatives (laser, die stamping, etc.)

The job will be awarded in a couple of months when final design work is complete.

Please provide a quote that includes pricing and leadtimes for:

10 initial samples (made from same process as production)
100 pcs (for initial testing)
1000 pc lots (production)

Material: ABS Sheet .031 thick
Color: Black
Texture: smooth / matt (will be covered up by a label)
Tolerance needs to be +/- 0.010 (may be able to relax somewhat based on final design)

Not sure what is up with the dxf export on this part???? Anyway, here is a PDF file that should suffice for quoting.
