RFQ - 2" Disk with Cut in Lettering

I need a 2" disk of just about any material that will give a smooth cut, preferably plastic. The disk is simply 2" diameter, 1/4" thick with 1/16" deep lettering cut in, 7 characters total.

I need it quick and preferably cheap.
