I make HDPE ones all the time. and using 1/4" bolts holes for adjustment. just state the sizes you need, or a drawing. you can message me.

I’m building my first CNC machine.
I’m placing a 5/8” steel rod through a ¾” plywood hole.
I need some kind of Adjustment block on the other end that has screws in it
so I can adjust the level of the rod. It would be a square block with 3
or 4 screws used to tighten and center the rod.

I think you're cutting the adjustment a bit tight. I used t-nuts on my adjusters so had about 1/2" around my pipes, but even without them I'd advise at least 3/8" around your pipe.
